Insurance Coverage and Payment Options
At Heart Health Center of North Texas, we accept a wide range of insurance plans, including Medicare, Medicaid, PPO, and HMO's. We also offer affordable self-pay options to ensure that quality cardiac care is accessible to all. New patients can conveniently have their primary care doctor fax a referral with insurance information to us. Self pay patients can call to schedule their visit.
New Patient Referrals
If you are a new patient and have a referring or primary care doctor, please have them fax a referral to us . Include your last progress note and your demographic information. Please make sure your primary care doctor has updated insurance information, correct address and phone number with voice mail for you. If you have a HMO insurance plan that requires approval from the insurance company, we will need an authorization number also. Your primary care doctor can help you with that.

Directions from Rockwall - I90 tollway
On I30 headed west towards Dallas exit N.W. Drive. Off of the I30 service road turn right into the parking lot after the big blue sign. We are located in the Women's Health Alliance building, across the highway from Lowe's.
Directions from Dallas - I30 - 635
Get on I30 headed east. Exit either N.W. Drive or Beltline Rd. Do a "U-turn" over the bridge and get on the I30 service road headed west towards Dallas. Off of the I30 service road turn right into the parking lot after the big blue sign. We are located in the Women's Health Alliance building, across the highway from Lowe's.
Directions from Republic Parkway - Galloway
On Republic Parkway turn right into the parking lot for 1600 and 1650. Drive all the way to the BACK behind the 1650 Dialysis building. Once you are in the back parking lot turn LEFT and follow the back parking lot around. You will pass the Rehab sign. Behind the trees you will see another building with the sign "Women's Health Alliance" on it. That is the building we are in. Circle around to the front, we are in ste 100.
